Patient registry and card basics
Visit a physician with a Certificate to Recommend (in person or telemedicine) at least once a year. The physician confirms a qualifying condition and enters the recommendation in the Patient & Caregiver Registry; the patient activates the account by email. Buying requires an active registry card, an active recommendation and a government ID. Registration renews yearly. Helpline 1-833-464-6627.
